Definitions-
Clinical safety concerns are defined by MTSA as:
- Reported behavior which puts a patient at risk for adverse events or poor outcomes
- Submission of a yellow card by a clinical instructor or facility
- Yellow cards indicate behaviors which may cause elevated and unnecessary risk
- Submission of a red card by a clinical instructor or facility
- Red cards indicate behaviors which do cause elevated and unnecessary risk
Protocol-
- Clinical safety concerns may be reported through Typhon, email, or other means of communication.
- The recipient of a clinical safety concern should ensure it is documented promptly. Also, the recipient should ensure notification of the Assistant Program Administrator, Clinical Education and the advisor for the student.
- All information will remain confidential and only be shared in accordance with FERPA regulations.
